What are the Documents used for?

  • Maternity Confirmation Letter – this letter acknowledges the pregnancy and is used to confirm commencement of maternity, entitlements to leave and pay and other related requirements, such as paid time off for antenatal care, risk assessments, keeping in touch and returning to work.
  • Maternity Leave Form – this form is used to confirm maternity leave dates, entitlements and annual leave before and after maternity leave. It should be completed by the employee and their Manager and submitted together with their MATBI Form by the end of the 15th week before the expected week of childbirth.
  • Maternity Risk Assessment including COVID example – you are obliged to complete a workplace risk assessment when an employee is pregnant and to regularly review and update this. This template provides a framework to enable you to do this.
  • Shared Parental Leave – this form can be used if the employee wishes to avail of Shared Parental Leave.
  • Parental Leave Form – this form can be used by an employee to request unpaid parental leave.
  • Dependent / Emergency Leave Request Form – this form can be used by an employee to request urgent and unpaid time off to deal with an emergency or unforeseen situation involving a dependant and to enable them to make any necessary longer term arrangements. Given the nature of such time off, it is noted that this form may be completed after the emergency leave has commenced.
  • Paternity Leave Confirmation Letter – this letter is used to confirm paternity leave on receipt of the SC3 Form.
